Sally A. Blonk, age 72, of Grand Rapids, passed away on Sunday, January 13, 2013 at Heartland Health Care Center. The daughter of Nicholas & Dorothy (Mussel) Hayes, she was born on June 6, 1940 in Grand Rapids, MI. Honoring her wishes, cremation will take place.
